Diagnosing polymyalgia rheumatica involves doing several tests:

A physical examination along with joint and neurological examination is done. These test results help the doctor to determine the cause of the pain and stiffness.

Blood tests such as erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) is done. Some people with polymyalgia rheumatica will have slightly high ESR and CRP or normal range of these levels.

Imaging tests such as ultrasound is done to distinguish polymyalgia rheumatica from other conditions that cause similar symptoms. MRI helps to identify other causes of shoulder pain.
