After a heart transplant, these steps are considered.

Hospital Recovery- The patient might recover from the hospital within two to three weeks. He or she spends the first part of the recovery in the ICU. The health care providers monitor the vital signs and other signs to know whether the patient’s body is rejecting the new heart.
Plan of Care- Before leaving the hospital, the doctor might explain the plan of care that a patient should follow while recovering at home.
Cardiac Rehabilitation- The doctor will tell the patient to join cardiac rehabilitation program, which is a prescribed monitored program that helps to improve strength, endurance and heart function.
